As part of getting children ready for school, Community Nursery Nurses complete another assessment with parents. It is carried out between the age of 2 and 2½ years. 

A letter will be sent to you containing your appointment details and two Ages and Stages Questionnaire (ASQ3 and ASQ SE) Parents are required to complete these before attending the appointment with the Community Nursery Nurse.

At the appointment the Community Nursery Nurse will review your child’s health, growth, wellbeing and development. Should there be any indications that your child may need further review or assessment, referrals will be completed.
