
As part of getting children ready for school, Community Nursery Nurses complete an assessment with parents. It is carried out between the age of 2 and 2½ years. A letter will be sent to you containing your appointment details and two Ages and Stages Questionnaire (ASQ3 and ASQ SE).
Parents are asked to take the time to complete these before attending the appointment with the Community Nursery Nurse.
At the appointment the Community Nursery Nurse will review your child’s health, growth, wellbeing and development. Should there be any indication that your child may need further review or assessment , a plan will be discussed with you. This may include support you can offer your child in the home, local groups and activities that may benefit your child, timescales for further assessment or what referrals might be considered, either now or in the future.
