Referrals

Referrals will be accepted from parents and guardians and professionals in health, education and social care. A specialist children's service referral form must be completed and signed consent from parent/guardian to share information must be provided. Referrals can be emailed or posted directly to the service or made through single point of access (SPA) using the address and contact information below:
