
The Child Development Nursing Team are skill mixed team and provide specialist support to children and young people aged 0-19 years who meet the criteria for referral to the Child Development Team (see referral criteria).
The Child Development Nursing Team supports the multi-disciplinary assessment, diagnostic and on-going management for children and young people who may have disability or developmental problems. They help develop and deliver care pathways for children with additional needs, offering advice, support, and assessment, working in partnership with the multi-disciplinary teams and partner agencies.
The Child Development Nursing Team also includes a Nursery Nurse who provides various play opportunities for children aged 0-5 years as part of the assessment and on-going support.
The Child Development Nursing Team works with children’s Therapists, Paediatricians, Health Visitors, School Nurses, Education, Social Services, and other agencies to provide a multi-disciplinary approach centred around the child or young person.
Children must be aged between 0-19 years of age and have complex health care needs with impairment in at least two areas of development e.g.
Referrals are received through the Children Single Point of Access. These referrals can come from any professional giving clear indication of what is required from the child development nursing team that universal services are unable to offer.
