Referrals can be made by telephone, but must be supported by a Community Children’s Nursing Referral Form. Please call above number for referral form

Referral Criteria         

• Resident in South Essex area with a GP within the South Essex area.
• Patients must be 0-19 years of age.
• Child must have a nursing need for the referral to be accepted.
