Where are we?:
All referrals go to the Single Point of Access (SPA).
Referrals are triaged there and are then tasked to us in Southend if the young people meet the threshold for the 'Getting more help' and/or the 'risk management and support' aspect of the iThrive model which underpins our care delivery.
Once a referral is accepted by Mid Essex, an Initial Assessment is offered with one of our clinicians. If the presentation is of high risk and urgency, an urgent initial assessment is offered within 2 weeks or as early as is possible.
This initial assessment allows us to be have sight of the presenting need, the contributing factors, risk profile and allows us to formulate an individualised care plan for the young person. This may also include the involvement of other services as we aim to work collaboratively with a multi- agency approach. No two people are the same and our team are trained to help people with a wide range of difficulties.
Within our team, we then provide therapeutic interventions and support based on the care plan formulation. This may be DBT, CBT, Systemic family work, Psychotherapy, EMDR, Trauma work, just to name a few.
There are regular reviews along the way, with the use of various outcome measures, that helps the young person as well as clinician to appreciate the progress (or lack thereof) of their recovery.
The discussion around Endings is had with the young person and when appropriate, we start the discharging/ transition process. We again, at this point of care, work with a number of other services to ensure continued support for our young people.
