Referrals

Referrals will be accepted via a GP or other healthcare professional.

Criteria:

• Type 1 diabetes - structured education only - DAFNE
• Newly diagnosed Type 2 diabetes - structured education - X-pert Programme
• Patient on maximum tolerated oral hypoglycaemia agent
• Patient for GLP-1 or insulin therapy
• Patients on insulin therapy who require optimisation of control

This service is available to residents of Waltham Forest and referrals are accepted via a GP or other healthcare professional requiring advice and education.
