Referrals

Referrals will be accepted from parents/guardians and professionals in health, education or social care. A specialist children's service referral form must be completed, and signed consent from parent/guardian to share information must be provided.

Criteria: Children and young people between 0-16 years old, depending on transition plans - some young people up to 19 years old. Children and young people living in Waltham Forest.
