Referral

Referrals are accepted from parents/guardians and professionals in health, education or social care. A specialist children's service referral form must be completed, and signed consent from parent/guardian to share information must be provided.

Criteria: children up to 19 years old, with a functional concern. Functional concerns include age inappropriate difficulties; using cutlery; dressing; riding a bike; participating in PE lessons; hand dexterity, for example building Lego or using scissors; children aged 12 and over with a terminal condition/newly acquired condition/in need of a hand splint. The primary reason for referral is for an occupational therapy assessment, therapy, programme and advice.
