Nutrition and dietetics

The team consists of HCPC Registered Dietitians, Dietetic Assistants and Administrators. The service delivers one-to-one care in a variety of ways. These include virtual clinics (telephone or video), face to face clinic appointments and group education subject to inclusion criteria. The service aims to improves the overall health of individuals needing special diets as part of medical treatment. Dietitians translate scientific information about food into practical dietary advice across the lifespan.

Services provided include:

  • Assessment and management for dietetic related conditions for both adult and children that meet the service criteria
  • Group Education Sessions for:
    — Weight reducing patients (Tier 2 weight management service only)
    — Pulmonary rehab sessions
    — Lipid lowering sessions
  • Structured group education for patients with diabetes:
    — Dose Adjustment For Normal Eating (DAFNE): This programme aims to help adults with type 1 diabetes.
    — Diabetes Education and Self Management for Ongoing and Newly Diagnosed  (DESMOND): This programme aims to help adults with type 2 diabetes
  • Appointments can be delivered at various clinical sites, nursing homes and home visits are available for house bound patients
  • MUST training for health professionals

Service Exclusions:

  • Parenteral feeding
  • Oncology patients undergoing treatment.
  • Gestational diabetes
  • Palliative patients who are < 3 months from end of life.
  • Tier 3 & tier 4 weight management
  • Malnutrition universal screening tool (MUST) score < 2 (NB, MUST score >2 with following conditions will be accepted: pressure injury, texture modified diet as seen by SLT, COPD)
  • Renal and liver disorders under specialist services
  • Eating disorders including ARFID
  • Paediatric diabetes
  • Paediatric immediate IgE allergies and multiple allergies

Referrals

Referrals will be accepted through single point of access (SPA) by email from consultants, GP practices and health care professionals (band 5 and above). Referrals are accepted for patients with a Havering GP. Please refer to the For Clinicians page for more information.
