Integrated care team

The integrated care (IC) team ensures patients over the age of 18 with complex health and social care needs receive the right care, in the right place and at the right time. The ICM team works closely with GPs and other health and social care providers to co-ordinate and offer a multi-disciplinary, high quality package of care to vulnerable adults.

The ICM team is divided into three localities across Havering, operating from local clinics and health centres.

For urgent matters out of hours service transfers to community treatment team (CTT) (Monday to Sunday 8am-8pm) on 07940 301396. Outside of these hours please contact the community night nursing service on 0300 555 1269.

Referrals

Referrals will be accepted form GPs and health and social care professionals. Self referrals will also be accepted from patients and clients.
