Referrals

Referrals will be accepted by faxing or posting the referral form or via SystmOne.

Criteria: Adults residing in south west or south east Essex except Brentwood.

Exclusions: Patients with the following conditions will not normally be seen but we can support in providing resources:

  • simple obesity BMI below 30kg/m2 or < 27.5kg/m2 (if South Asian or Chinese) with no other comorbidities
  • simple constipation
  • gout
  • hyperlipidaemia if patient on medication
  • indigestion or migraine
  • on End of Life pathway
  • poor appetite where BMI is >18kg/m2 with MUST score of 2 or less
