Referrals

Referrals will be accepted via SystmOne or by emailing the completed referral form to: essextissueviabilityservice@nelft.nhs.uk Telephone advice is also available on the number above.

Criteria:

• Adults over the age of 18 who are managing a chronic wound that is non-healing, including: leg ulcers, pressure ulcers, surgical wounds, diabetic wounds
• Living in Basildon, Brentwood or Thurrock
• Registered with a GP in Basildon, Brentwood or Thurrock
