Referrals

Referrals will be accepted via electronic means on SystmOne or by telephone (on above numbers) Self-referrals will also be accepted but please note this is only if the patient is already registered with the service. For patients not currently known to the service, referrals must be made via a health or social care practitioner.

Criteria: Housebound adults over the age of 18, requiring universal nursing care or short term therapy rehabilitation, living in South West Essex who is registered with a South West Essex GP
