The CGS team provide all patients with a Comprehensive Geriatric Assessment (CGA), designed to determine a frail older person’ medical, mental health, functional capacity and social circumstances.

The service aims to enhance the quality of life for people with long term conditions by helping people to recover from injury or episodes of ill health. We also aim to reduce the need for unnecessary emergency admissions. We provide provide comprehensive assessments for older people in the community and support to care homes. We use a co-ordinated case management approach.

Referrals

Referrals will be accepted from GPs, NELFT nurses and allied health professionals (AHPs), via SystmOne or by faxing or posting the referral form to the single point of access (SPA).
