Referrals

Referrals are accepted via telephone triage on 0300 300 1836

Criteria: Age 18 and over with an intensive rehabilitation need and registered with a GP in Barking & Dagenham, Havering or Redbridge.

Exclusions: Acute (new) stroke diagnosis. Patients who are unlikely to tolerate intensive daily rehabilitation and those without an intensive rehab need supported by SMART goals.
