Referrals

Referrals are made by GPs and health and social care professionals. Self referrals can be made by patients and clients.

The Integrated Care Team (ICT) ensures patients over the age of 18 with complex health and social care needs receive the right care, in the right place and at the right time. The ICT works with health and social care providers to co-ordinate and offer multi-disciplinary quality care to vulnerable adults. The ICT is divided into three localitites (east, west, north) across Barking and Dagenham.
