The JRT aim to prevent hospital admissions and facilitate earlier and safe discharge from hospital. They enable people who have had a crisis to return home or stay at home and maintain their independence.  They will work to develop confidence and skills to carry out daily living activities and continue to live safely and independently at home.    Each service user is provided with an outcome-based reablement plan to meet their needs.
