Referrals

Referrals are accepted from parents/carers, school nurses, teachers, social services and specialist children's centres. Young people can also self refer.

Referral Form:

Criteria: Aged up to 18; Young person must know that they are being referred and consent to referral

Exclusions: Cannot be referred if they are under an order from a youth offending team.
Test results required at referral: None
