Referrals

Referrals will be accepted by healthcare professionals and GPs, through single point of access (SPA) or self-referral direct to the service.

Criteria:

Patients over the age of 18.
Confirmed diagnosis of Parkinson's Disease or Multiple Sclerosis.
GP within the borough of Waltham Forest.
